To ensure your protection and ours, kindly fill out the photo release form which lists the term and conditions for the use of your photos and for the in-kind compensation you will receive in the form of a 1 on 1 Reiki session for 1 hour.

This Photo Release Form (the "Agreement") is made by and between:

Model (whose personal and contact information is provided below)
Description of Photographs: The photographs taken will include images of the Model participating in Reiki and Yoga sessions. These images may be used for website, social media, YouTube, and other commercial purposes related to [Your Name/Your Business Name]'s Reiki and Yoga business. The Model agrees that he or she could be photographed with other models and with the business representative depending on the required photo. Video may also be taken.
Grant of Rights: I, the undersigned Model, hereby grant to [Your Name/Your Business Name] and its legal representatives, successors, and assigns, the irrevocable and unrestricted right to use, reproduce, and publish photographs of me, or in which I may be included, for editorial, trade, advertising, and any other purpose and in any manner and medium, including, without limitation, use on websites, social media platforms, YouTube, and other commercial purposes related to [Your Name/Your Business Name]'s Reiki and Yoga business.
Model's Representations and Warranties: The Model hereby represents and warrants that:
The Model is of legal age and has the full legal capacity to execute this Agreement.
The Model has read this Agreement and fully understands its contents.
Compensation: The only compensation for the Model will be a voucher for one 1-hour Reiki session to be performed by a representative of Light and Balance Reiki and Yoga at an agreed-upon time within 365 days of the signing of this Agreement and upon receipt of the photo shoot services. The Model must complete the shoot successfully, which will last no more than 2 hours on the agreed-upon date. The compensation of the 1-hour session can be transferred to a person of the Model's choice who will also be bound by the same conditions of reiki session services. The Model or recipient of the treatment must fill in the necessary intake forms and sign the requested liability and waiver forms. No monetary compensation or royalties will be provided.
Professional Conduct: The Model agrees to behave professionally and respectfully during the photo shoot, as does the business representative of Light and Balance Reiki and Yoga.
Revocation of Consent: The Model understands that they can revoke this consent at any time by providing written notice to Light and Balance Reiki and Yoga. However, such revocation will not apply to photographs already published or in use, and a refund of the reiki session will have to be repaid to Light and Balance Reiki and Yoga at the posted rate in effect on the date the revocation is received and acknowledged by the business.
Indemnification: The Model agrees to indemnify and hold harmless Light and Balance from any claims, damages, or liabilities resulting from or related to the use of the photographs as described in this Agreement.
Governing Law: This Agreement shall be governed by and construed in accordance with the laws of the [State/Province] of [Your State/Province], without regard to its conflict of laws principles.
Entire Agreement: This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior agreements, understandings, and representations, whether written or oral.
Digital Signature: The parties agree that a digital signature shall be considered as valid as an original signature and shall be effective to bind the parties to this Agreement.
Signatures: By digitally signing below, the parties agree to the terms and conditions of this Agreement.
